The singing competition judge lost more than 50 pounds to help with his type 2 diabetes.
His current appearance stands in stark contrast to the man who was introduced to American Idol viewers in 2002.
Although he has been significantly thinner for many years, fans are still surprised by his transformation.

Randy was diagnosed with type 2 diabetes in 2002 and underwent gastric bypass surgery the following year.
Kelly, 53, and Mark, 52, did not speak about Randy’s health during the reunion.
Before surgery, Randy weighed 350 pounds.
He spoke candidly with fans about his struggles, detailing the challenges he faced due to his weight in his 2008 self-help book, Body With Soul.
“It is a curse to be confronted with a life-threatening illness that you cannot completely get rid of, although you can certainly control it,” the star wrote.
“But it’s a blessing to get this huge wake-up call.”
He continued, “After that day in the emergency room, when my doctor burst the bubble I was living in, I couldn’t like myself anymore.”
“At that moment, I began my journey toward better health.”
Fans first noticed Randy’s weight loss in May 2021.
He appeared alongside Paula Abdul, serving as judges after American Idol alumni including Laine Hardy, David Cook and Jordin Sparks performed duets in honor of the show’s 20th anniversary.
Paula, 62, and Randy were judges on the first few seasons of American Idol alongside Simon Cowell.
