During the family getaway, Kourtney posed with her younger siblings, Khloe and Kim Kardashian, 43.
The trip felt carefree on the tropical beach as they enjoyed the sun and the crystal clear ocean behind them.
Each wore a tiny black bikini that varied slightly in style.
In the photo, Khloe was wearing the same sarong over her bikini bottoms.
“Happy birthday, queen [Kourtney]! There is no one on this planet that I have spent more time with and had the same memories of growing up than you and I cherish them all,” Kim captioned the lengthy post.
“Sharing a room, friends, cars and our lives together for four decades has been the most magical journey to have you by my side. I love you and can’t wait for 45 more years together!”
“I will especially cherish this last sisters trip, where all of our kids had dance parties until they passed out, like we all did growing up!” she added.
