A MAN had to have part of his penis amputated after a sex toy “strangled” his member and left it blackened and gangrenous.

The 68-year-old American was stuck wearing a ring to enhance his erection for a whole day before starting to remove it himself with pliers.

A graphic article published in Urology case reports details the doctors’ painstaking efforts to save the man’s blackened limb, before making the decision to remove the tip.

In what appears to be a world first, surgeons used part of a man’s thigh to reconstruct his penis.

Known as cock rings, tension rings and constriction rings, the solid or elastic instruments are placed around the base of the penis or testicles, slowing blood flow.

They can help make erections harder and longer lasting and can make sex more pleasurable for some.

But it is advised that users do not keep them on for more than 30 minutes at a time and that they ensure the rings are snug.

You should remove the cock ring immediately if you experience any pain, numbness, tingling, unusual swelling or bruising, Superdrug Online Doctor warn.

Keeping constriction rings on for a long period of time can put you at risk of gangrene – when the tissues in the penis can’t get enough oxygen and die due to their blood supply being cut off.

The 68-year-old man in question told doctors at the University of California’s urology department that he wore the ring for 24 hours, resorting to using pliers to remove it when he was unable to remove it by hand.

But the patient only sought emergency care a week later, after having difficulty peeing.

He still suffered from pain and swelling in his penis.

The man was seen by the urology team four days later and doctors recorded that his penis appeared abnormally dark and blistered.

After inserting a catheter to help him pee, they deemed the patient to be in stable condition and discharged him to a ward.

However, this was not the last time they saw him.

The patient, who was homeless at the time, returned to the clinic two weeks later.

This time, his penis was black from root to tip, indicating that gangrene had set in after prolonged use of the cock ring three weeks earlier.

Doctors immediately administered antibiotics to the man in an attempt to stop the tissue necrosis.

The man was rushed into surgery at the same time. next day, where surgeons began the painstaking process of peeling away dead, blackened flesh until they found healthy tissue.

They made the decision to completely remove the head of his penis after finding it covered in “thick, woody” black tissue that could not be saved.

After a week-long recovery, the patient had more dead tissue removed before doctors turned their attention to reconstructing his penis.

They removed an eight by 13 cm (three by 5 inch) piece of skin from his right thigh, grafting it around the man’s body.

“To our knowledge, this is the first description of the successful use of this technique for tissue loss following debridement of penile necrosis secondary to the use of a constrictive ring,” the doctors wrote.

They reported that the man remained stable after surgery and about 85% of the skin graft ended up adhering to the penis.

He had a new catheter inserted into his abdomen to help him urinate – normally they are inserted through the penis – which appeared to be working properly.

The man has been discharged back to a ward but will be closely monitored with routine clinic visits, doctors added.

Reflecting on the case, the report’s authors wrote: “Penile strangulation by a constrictive ring is a urological emergency that warrants immediate removal to avoid vascular compromise and subsequent necrosis, as well as acute urinary retention.

“The management of these patients represents a clinical challenge given the availability of tools available to the urologist at their respective facilities to remove the constrictive device.”
