Simon Cowell’s face shows signs of eyelid lift and ‘pillow face’, says surgeon – after star admitted to ‘too much’ filler

SIMON COWELL hasn’t exactly been discreet about his plastic surgery or filler use.

According to an Upper East Side plastic surgeon, though, the America’s Got Talent judge probably got more cuts than he admitted.

Fans have certainly speculated about Simon Cowell’s changing face in recent years.

The 64-year-old got a “line lift” and went “too far” with fillers after viewers commented on his appearance on social media.

“Simon Cowell looks more like a Simon Cowell wax figure than a Simon Cowell wax figure,” wrote one Twitter user in 2019.

“It looks like Simon Cowell, someone tried to play Simon Cowell in The Sims 3, but got stuck playing with the eye buttons for too long,” joked another.

“Simon Cowell’s new face looks like a doll that you leave in the sun for too long and starts to melt,” wrote a third.

But while commentators see the overall change, one professional has detailed exactly what likely happened.

Double board-certified facial plastic surgeon Dr. Konstantin Vasyukevich told The US Sun that he sees signs of filler, eyelid lift and possible “pillow face.”

FLASHBACK TO THE 90’S

In 1994, Simon was still years away from becoming a household name, but he was already working in music.

His biggest claim to fame at the time may have been producing the Power Rangers theme song.

A photo of the then-35-year-old on the red carpet at the BRIT Awards shows off his “natural features,” according to Dr.

He said the music executive had a “very good chin and cheeks that weren’t particularly full.”

Simon didn’t look much different when he made his American Idol debut in 2002.

FACELIFT HELL

His public presence continued to grow over the next two and a half decades with America Idol, The X-Factor and America’s Got Talent – but it wasn’t until the late 2010s that fans began to notice he looked a little different.

The sun reported in 2018 that he had undergone a $2,550 “Silhouette Soft Lift” with Dr. Jean-Louis Sebagh, known as the “Picasso of dermatology.”

The procedure involves pulling a bioplastic-infused thread that has been sewn onto the face to eliminate sagging skin.

Simon admitted to going under the knife, complaining that it “hurt like hell”.

“There are many things you can do now. You don’t just have to fill your face with fillers and Botox,” he told The Sun on Sunday when seeing his doctor, who has a six-month waiting list.

“It hurts a lot, but it eliminates sun damage and disconnects all the crap.

“For me now, what matters is having clean skin. If you have clear skin, you look better.”

POWERFUL FACE SHAPE AND MORPHING

In 2019, speculation really increased on social media.

“I would like to personally thank Simon Cowell’s plastic surgeon for sewing his eyes shut upside down,” one person tweeted.

“I can’t get over the way Simon Cowell looks now, he’s certainly just a totally different person,” said another.

But Dr. Vasyukevich said there are some differences that only a professional would notice.

“Perhaps it is a natural result due to weight gain, but her cheeks appeared higher and her overall face shape and features are slightly pronounced,” he said, analyzing photos from 2019.

“You can see that he has lost volume in his face and a general decrease in the roundness and definition of his face.

“However, the cheeks were still very prominent and protruding.”

He added that “something seemed incongruous,” which could be a sign of fillers, but “fillers, in and of themselves, are neither good nor bad.”

Dr. Vasyukevich noticed that Simon’s forehead had fallen and was closer to his eyes, which could have been a natural effect of aging or Botox to relax the muscles.

“He had a skin fold on his upper eyelid, but as the eyebrow lowered the fold disappeared, so it could have been a blepharoplasty,” explained the doctor.

Blepharoplasty is an eyelid lift.

‘TOO FAR’ WITH FILLERS

Vasyukevich noted that over the course of two years, from 2019 to 2021, the overall volume of the AGT judge’s face increased.

“At this point, there may be filling in several areas,” he admitted.

“In addition, the forehead and cheeks had a bulge that could be the result of filler.

Between 2022 and 2023, he added that there was an overall “irregular appearance and bumps,” which he speculated were due to old infill and migration.

Former American Idol judge admitted to The Sun In 2022, he had gone “a little too far” with facial fillers and thought it looked like “something out of a horror movie.”

“There was a stage where I might have gone too far. I saw a ‘before’ photo of me the other day and at first I didn’t recognize it as me,” he told The Sun.

More recent photos, from the end of 2023 to today, “show a less puffy face”, with more wrinkles and an overall more natural appearance.

Cowell said he already had filler but dissolved it.

Signs of ‘bad work’

Konstantin Vasyukevich shared telltale signs of poor cosmetic work.

  • Unnatural appearance: “That’s the opposite of what we’re after,” he said.
  • Loss of facial expression: “A skilled surgeon understands facial anatomy and avoids procedures that restrict natural movement,” he explained. “If someone has difficulty smiling or frowning freely, this may indicate that underlying muscles or tissues were compromised during surgery.”
  • Asymmetry: “The human face is naturally asymmetrical, but a skilled surgeon seeks a balanced result,” he said. “Obvious asymmetry, such as uneven eyebrows, a crooked nose, or mismatched lips, can be a sign of poor technique or inexperience.”

Dr. Vasyukevich said the visual show above “may line up with him dissolving the filler.”

He has “good overall facial volume” but “less soft tissue laxity,” which may be due to other filler work that has diminished in spots.

PILLOW PROBLEMS

Even after the filler has dissolved, some people are left with an undesirable phenomenon called Pillow Face – and Dr. Vasyukevich sees signs that Simon falls into this camp.

He described this as an “uneven distribution of volume” that causes a loss of definition in the face.

There are three prominent signs that the average person can spot.

  • Cheeks that are too full: The cheeks become excessively fleshy, losing their natural contours and blending into the area under the eyes.
  • Flattened middle face: The natural creases and shadows of the middle face disappear, creating a soft, almost featureless appearance.
  • Smaller eyes: Because the cheeks are so puffy, they can push against the lower eyelids, making the eyes appear smaller.
