The list of reasons three Miami-Dade plastic surgeons were disciplined includes illegal surgeries, unlicensed personnel caring for patients after surgery and hiding patient records from state inspectors.
Regardless of whether these doctors committed these violations or not, they are responsible as designated physicians for the office-based surgical centers in Coral Gables, Aventura, and West Miami-Dade.
The designated physician of an office-based surgery center ensures that the center complies with regulations and laws. When Florida Department of Health inspections reveal violations at the surgery center, separate administrative complaints are filed against the surgery center and the assigned physician.

Keith Berman, Doll Plastic Surgery, Adventure
Keith Berman is still the designated doctor at Aventura’s Dolls Plastic Surgery, 18205 Biscayne Blvd., as he was when Florida Department of Health inspectors showed up on Feb. 22, 2023. The administrative complaint said inspectors found:
At least once, Dolls did not use complete preoperative evaluation forms that showed the surgeon had completed all steps of the preoperative evaluation.
At least once, there was liposuction where more than 4,000 cubic centimeters of supernatant fat were removed.
At least once, Dolls had a “major liposuction” using more than 1,000 cubic centimeters of supernatant fat “at a remote location from another procedure.”
Berman was fined $5,000, ordered to pay $2,180 in Department of Health case costs and ordered to take two five-hour continuing medical education classes on risk management and laws, rules and ethics.
Dolls’ corporate registration says it is run by businessmen Leonardo Campo and Heriberto Martinez.
Gamaliel Mattos, Xiluet Plastic Surgery, West Miami-Dade

Additionally, “the facility was not Level II or Level III compliant, requiring anesthesia and emergency training.” The Florida Department of Health says during a Level II office surgery, the patient may “tolerate unpleasant procedures” but remains conscious enough to respond to tactile or verbal requests. Level III office surgeries involve general anesthesia and preoperative sedation.
Inspectors also “did not find any policies and procedures manual, which must be kept in the office and updated annually.”
There was “no mandatory risk management program, including quarterly review of such information by the surgeon.”
Dr. Gamaliel Mattos, designated physician, was responsible for maintaining the transfer agreements and hospital staff privileges of the Xiluet surgeons. Mattos was also responsible for maintaining paperwork that showed Xiluet’s patients were informed of their surgeons’ transfer arrangements and staff privileges.
Mattos was fined $10,000, charged $2,096 in Department of Health case costs and ordered to take a five-hour continuing medical education course on risk management and another five-hour course on laws, rules and ethics. Xiluet is led by President Monica Vazquez.
John Sampson, Seduction Cosmetic Center, Coral Gables
Dr. John Sampson is no longer listed on the Seduction website or among the Seduction professionals on the Florida Department of Health website.
But they are still linked as defendants in the lawsuit brought by the estate of Erica Russell, who died on June 16, 2021, as the sixth or seventh seduction surgery of the day by Sampson, who was not supposed to be performing surgeries. .
Seduction and Sampson told the state he would only be the designated doctor. As designated doctor, Sampson was fined $5,000, plus $1,262 in case costs for Seduction violations. Additionally, a reprimand was issued against Sampson’s license.
These violations stem from a Department of Health inspection two days after Russell’s death. Inspectors found:
Doctor performing surgery who was not registered with the Department of Health to perform surgery in an office.
Doctors listed in the registry who no longer worked at Seduction.
At least one “post-operative follow-up…was performed by an unlicensed staff member who signed in as a nurse or doctor.”
“Multiple surgical records” without mandatory information such as time of arrival in the operating room, level of surgery, type of anesthesia, duration of recovery, recovery team, total supernatant fat removed, and adverse events.
Seduction, run by president Gretel Jardon since 2008, was unable to demonstrate that it had carried out a backup power safety check.
